Guiding an Improved Dementia Experience
At Gilchrist, we understand the journey with dementia can feel overwhelming. We are honored to have been chosen by the Centers for Medicare & Medicaid Services (CMS) to participate in the national GUIDE Program. This program provides extra support for people living with dementia and the families who care for them. Our goal is simple: to ease the challenges of dementia and help families focus on what matters most—to Live Every Moment.
What Gilchrist offers through the GUIDE Program
- Personalized Care Plans tailored to each person’s needs and goals
- Dedicated Care Navigators to guide families and connect them with resources
- Education & Trainings so caregivers feel more empowered and confident in daily care
- In-Home Respite Services for those who qualify
- Access to 24/7 Support
- Advance Care Planning to ensure wishes and known and respected
Who Can Join
You or your loved one may be eligible if:
- Enrolled in Medicare Part A & B (Advantage Plans are not eligible)
- Have a diagnosis of dementia
- Live at home or in a community setting (non-nursing home)
- Not enrolled in hospice or PACE program
How to Get Started
- Contact Us: Call 888.823.8880 or complete the Referral Form and a team member will be in touch.
- Assessment: Our team will review your needs and create a care plan if eligible.
- Ongoing Support: Once enrolled, you’ll have a Care Navigator walking alongside you and your care team.
