This certification allows patients and families to have confidence that their end-of-life heart failure care is expertly coordinated and aligned with the American Heart Association’s rigorous, science-based guidelines.

What this means for you

Heart disease is the leading cause of death in the United States, yet only 5% of people with end-stage heart failure receive hospice care—care proven to improve comfort, quality of life, and reduce hospitalizations.

Gilchrist supports nearly 1,000 hospice patients each year whose primary diagnosis is heart failure. Earlier referrals allow patients to experience better symptom control and fewer hospital visits, helping them live every moment to the fullest.

Heart disease often brings unmanaged symptoms and repeated hospitalizations. Even though it causes one in every four deaths, many individuals with advanced heart failure do not receive the hospice or palliative care that could ease their journey.

When the illness can no longer be cured or effectively controlled, Gilchrist’s expert team provides the support patients need to remain at home. Our care includes:
