Luminis Health Gilchrist Lifecare Institute: This partnership provides an integrative continuum of care for the elderly across multiple settings, including hospitals, doctors’ offices, long-term care facilities and at home. Services include geriatric supportive care at home; geriatric supportive care in residential communities; and hospice care.  Services are offered in Anne Arundel and Prince George’s County.

In our most recent survey, Gilchrist achieved CHAP certification for its palliative program and hospice care, marking a remarkable achievement. We not only met but surpassed the highest levels of excellence in every category assessed by CHAP surveyors. This distinction demonstrates our commitment to continuous improvement, ensuring that you receive the best care possible. Hospice Honors Award Recipient

The prestigious Hospice Honors Award celebrates Hospice Consumer Assessment of Healthcare Providers and Systems (HCAHPS) satisfaction and quality measures. To be considered for the Hospice national award, the hospice agency must have scored above the HEALTHCAREfirst National Performance Score on the Hospice CAHPS Willingness to Recommend question. If that criterion is met, performance is evaluated utilizing a set of 23 additional quality indicator measures.    This award underscores our unwavering commitment to ensuring the comfort and dignity of our patients throughout their journey. Hospice Honors recipients are leaders in delivering quality care, constantly seeking ways to enhance the patient’s experience.
