RN Care Manager
The RN Care Manager is a licensed Registered Nurse responsible for delivering compassionate, comprehensive hospice care within facility settings. This role involves conducting initial assessments, developing and implementing individualized care plans in collaboration with the interdisciplinary team, and providing direct nursing care to patients. The RN Care Manager maintains open communication with facility staff and families, attends care plan meetings, reconciles medications, and ensures continuity of care through detailed documentation. They educate facility staff and caregivers, supervise hospice aides, coordinate with physicians and clinical managers, and support patients and families during end-of-life care. Additionally, the RN Care Manager participates in marketing and customer service initiatives alongside the Business Development Manager, fostering strong professional relationships across all levels of facility staff.
Business Development Manager
The Business Development Manager builds and maintains strategic relationships with physicians, hospitals, and residential care communities to promote hospice services, drive earlier referrals, and grow census. This role involves developing and executing targeted sales plans, conducting educational presentations, and coordinating with internal teams to expedite admissions. The manager gathers and analyzes referral data, resolves customer concerns, and contributes to marketing initiatives and quality improvement efforts. Strong communication, customer service, and sales skills are essential, along with three years of healthcare experience and proficiency in CRM and EMR systems.
Hospice Admissions Nurse
The Hospice Admissions Nurse conducts comprehensive assessments of patients in hospitals or home settings to determine hospice eligibility and initiate care. Serving as a clinical ambassador for Gilchrist Inova, this nurse develops initial care plans, provides direct patient care, educates families and caregivers, and coordinates necessary equipment and services. The role requires strong clinical judgment, documentation skills, and the ability to collaborate across departments and with referral sources. A current Virginia RN license and three years of nursing experience are required; hospice or home health experience is preferred.
Hospice Aide
The Hospice Aide provides essential personal care and support to hospice patients under the supervision of a Registered Nurse, following the individualized Hospice Aide Plan of Care. Responsibilities include assisting with daily living activities, monitoring vital signs within training limits, maintaining a clean and safe environment, and offering emotional support to patients and families. The aide also helps with meal preparation, light housekeeping, and respite care, while observing and reporting changes in the patient’s condition. This role requires current Virginia CNA certification, a valid driver’s license, and reliable transportation.
Access Coordinator
The Access Coordinator manages all aspects of the hospice referral and intake process, ensuring timely, compassionate, and professional service delivery. This role involves scheduling new patient admissions, coordinating nursing and support services, and maintaining accurate documentation in Epic and other systems. The Access Coordinator communicates effectively with patients, families, referral sources, and vendors, while verifying insurance and physician credentials. Strong customer service, attention to detail, and proficiency in EMR systems are essential. One year of healthcare experience and prior customer support experience are required; LTC experience is a plus.
Hospice Clinical Manager
The Hospice Clinical Manager oversees the delivery of patient care services, managing clinical operations, staffing, and resource allocation to ensure high-quality, compassionate care. This leadership role involves strategic planning, financial oversight, staff development, and performance management. The manager monitors patient outcomes, ensures regulatory compliance, and drives quality improvement initiatives. Strong communication and coordination skills are essential for managing interdisciplinary teams and after-hours care. A current Virginia RN license, a bachelor’s degree (master’s preferred), and at least five years of progressive clinical experience, including two in a supervisory role, are required.
Clinical Social Worker
The Clinical Social Worker provides compassionate psychosocial support to hospice patients and their families, conducting assessments, developing care plans, and offering counseling around illness, coping, and end-of-life planning. As a key member of the interdisciplinary team, the social worker connects families to community resources, facilitates advance care planning, and participates in bereavement care. This role requires a master’s degree in Social Work, Virginia licensure (LMSW, LCSW, or LCSW-C), strong communication and organizational skills, and the ability to travel to patient homes. Hospice experience and cultural humility are highly valued.
